Reports of (actual, alleged or suspicious):
·  Verbal Abuse
·  Physical Abuse
·  Sexual Abuse
·  Neglect
·  Misappropriation
·  Exploitation
·  Any Death
(suspicious cause or not) / Notify the Intake IA via telephone or in person

Email: / Allegations are always filed as an MUI regardless of DD employee** involvement at the time of the incident
The provider must take all reasonable measures to ensure health and welfare of at-risk individuals
The provider must notify law enforcement, children services, SSA and the individual’s guardian as applicable the day the incident is discovered
Reports of:
·  Rights Violations
·  Prohibited Sexual Relations
·  Failure to Report / Notify the Intake IA via telephone or in person
Notify on-call personnel after business hours / Submit your written incident report to the Intake IA via fax, email, or in person / MUI is filed only when the PPI is a DD employee
Reports of:
Peer to Peer Acts
·  Exploitation
·  Physical Act
·  Theft
·  Sexual Act
·  Verbal Act / Notify the Intake IA via telephone or in person
Notify on-call personnel after business hours / Submit your written incident report to the Intake IA via fax, email, or in person / MUI is filed for all individuals involved
Actual Occurrences of:
·  Law Enforcement (individual is arrested, charged or incarcerated)
·  Attempted Suicide / Notify the Intake IA via telephone or in person
Notify on-call personnel after business hours / Submit your written incident report to the Intake IA via fax, email, or in person / MUIs are ALWAYS filed for these categories regardless of a DD employee/provider involvement at the time of the incident
Actual Occurrences of:
·  Missing Individual
·  Medical Emergency
·  Unscheduled Hospitalization
·  Significant Injury
·  Unapproved Behavior Support / Notify the Intake IA via telephone or in person
Notify on-call personnel after business hours / Submit your written incident report to the Intake IA via fax, email, or in person / MUIs are filed only when a DD employee/provider is present at the time of the incident
**UBS MUIs are only filed when the person utilizing the UBS is a DD employee

**A DD employee is anyone paid to provide services to an individual with DD
