A brief overview of health office guidelines:

§  All medications with the prescription/authorization note must be brought to the health office, including self-administered medications for approval.

§  Health forms should be submitted to the health office.

§  Any student coming to the health office should come with the teacher’s permit using the student’s planner, or referral slip from the department.

§  Teachers requesting Band-Aids to their class should send students with a written note.

§  Escort seriously sick students to health office or call ex 88460.

§  Students should eat their breakfast in the cafeteria before the first period/class instead of hiding in the health office for their meals.

§  Please, encourage students to call the school nurse when sick or visit the health office before they call parents, guardian, or ambulance.

§  The health office is closed daily for nurse lunch between 12:15 PM-1:00PM; during that time please see someone in the student support office.

§  Please, let us be mindful what meals, snacks we share with students in the classrooms; we have almost fifty-six (56) allergy lists so far with serious consequences if they break the rules……

§  All students for sports should submit physical exams to their PE teachers and a copy to the health file. Please, remind the student athletes that before participation in any sports, a pre-participation physical examination (PPE) is recommended and required for those engaging in competitive sports.

The goals of this evaluation are to:

1.  Assess the athlete’s present fitness level

2.  Detect any congenital and acquired condition

3.  Evaluate any existing injuries or illness of the athletes

4.  Meet administrative or legal requirements of the state or school district
