This form provides permission for the use of images of the child/young person to be used. Please note if you have more than one child under the age of 18 registered with the club you will need to complete a separate form for each young person.

Elland Junior FC recognizes the need to ensure the welfare and safety of all young people in football. As part of our commitment to ensure the safety of young people we will not permit photographs, video or other images of young people to be taken or used without consent of the parent/carers and the young person.

Elland Junior FC will follow the guidance for the use of images of children and young people, a copy of which are below. (Ten Golden Rules)

Elland Junior FC will take steps to ensure that these are used solely for the purpose they are intended which is the promotion and celebration of the activities of Elland Junior FC.

If you become aware that these images are being used inappropriately you should inform Elland Junior FC Child Protection Officer John Sylvester immediately.

The information will be available on the website for the football season 2008/09 after which it will be removed.

If at any time either parent/carer of the young person wishes the data to be removed from the website, seven days notice must be given to the Child Protection Officer (John Sylvester 07947724660)

Ten Golden Rules To Remember

1All children must be appropriately dressed

2Photography or recording should focus on the activity not on a particular young person

3Images should focus on small groups rather than individuals

4Images of a child who is under a court order should never be used

5If a young person is named avoid using their photograph

6If a photograph is used avoid naming the person or use their first name only. Personal details should NEVER be revealed

7Make sure parents/carers/young persons have signed and returned the parent/carer and young person permission form

8Use photographs that represent the broad range of youngsters participating in football

9All people taking photographs or recording footage at a football event should register with the event organizer

10All concerns regarding inappropriate or intrusive photography should be reported to the appropriate organization
