This memo serves as a written warning for your actions on _____________. (Describe specifics of unsatisfactory performance or behavior since the last disciplinary action.) When your colleagues asked for your assistance with a report you had previously worked on, you raised your voice and questioned their competency in front of several people. This kind of behavior will not be tolerated.

(Describe supervisor’s recent efforts to discuss, address or correct issues. Describe the impact of this unsatisfactory performance on the work, co-workers, clients, etc.)You have been counseled two previous times this year, once in February and once in May for abruptness in your interactions with your colleagues. Those instances were noted in your June 2011 performance evaluation. The department arranged for you and your colleagues to attend effective communication training in July, apparently to no avail. Your lack of cooperation and civility continue to jeopardize work relationships and disrupt productivity.

(Expectations & consequences going forward.)Yesterday’s event illustrates that more must be done by you to correct your behavior. You will meet with me tomorrow and have a plan to address your outbursts and inappropriate comments and to restore the eroded trust between you and your colleagues. There must be immediate and consistent improvement in your communication and interaction with colleagues, supervisors and visitors to this office. Another outburst or inappropriate comment will result in further discipline—up to and including termination of your employment.

Should you wish to appeal this decision, you have the right to resolve this matter in accordance with the University of Nebraska at Omaha Employee Handbook.
