T2P
The AAFP is piloting a program, called Translation to Practice, T2P. Each conference is eligible for an additional 2 credits by stating what you learned and how you plan to implement the newly acquired skill. This program bridges the gap between the classroom and the clinical setting. Any conference lecture is eligible for up to 2 additional FREE CME prescribed credits, by documenting how you will apply what you’ve learned to your practice.

What you need to do:

STEP 1: Complete the Commitment to Change Statement (on reverse side) for each session

1. Summarize what you learned in the CME activity-- Title of lecture _______________________

2. Identify the change you plan to integrate into your practice

3. Clarify your motivation for implementing the change

4. Check what you think will be improved Knowledge Competence Performance Patient Care

STEP 2: Send your commitment to change via email to no later than June 8, 2013.

STEP 3: Complete AAFP Translation to Practice reflection summary. NYSAFP will contact you with this, 90 days (September 9, 2013) following the submission of the Commitment to Change statement. You will confirm implementation of the change(s) identified in Step 1 by completing a reflection. The reflection will include identifying:

· the change you made

· the effect of the change on your practice,

· where you obtained information to implement the change,

· whether you encountered any barriers to change and

· what you did to overcome those barriers.

Send your reflection to no later than October 9, 2013.


Step 4: NYSAFP reports the t2p credit to the AAFP for you and you have 2 extra credits on file!
