HEALTHCARE COORDINATOR I
REPORTS TO: MANAGER OF HEALTHCARE COORDINATION
The Healthcare Coordinator I is focused on the deliberate organization of patient care activities between two or more participants involved in patient care in order to facilitate the appropriate delivery of health care services, and minimize the dangers of fragmentation. The Healthcare Coordinator I’s efforts are focused on patients going outside the patient-centered medical home for care and special populations in need of surveillance monitoring and timely reminders for disease prevention services. The Healthcare Coordinator I collaborates with providers, patients, and community partners in the medical neighborhood.
1. High school diploma or GED
2. Experience in the medical or health insurance field preferred
3. Knowledge of medical terminology
4. Excellent customer service skills
5. Excellent telephone etiquette
6. Fluency in written and spoken English and Spanish, including medical terminology, preferred
7. Excellent interpersonal skills, including friendliness, empathy, patience, kindness, politeness and helpfulness
8. Ability to build rapport with community partners in the medical neighborhood
9. Ability to work independently with a strong sense of focus
10. Task-oriented, strong organizational skills, ability to multitask
11. Strong attention to detail
12. Skilled in basic computer operations and EMR
1. Assess patient and family preferences for method of receiving information
2. Communicate information, including but not limited to appointment details and health reminders, to patients via telephone, mail, or other available communication tools
3. Ensure that barriers to care (such as language, transportation and financial) are addressed
4. Collaborate with available community programs, or internal departments to provide assistance to patients
5. Provide clear, thorough and accurate documentation of all interactions with patients, and other individuals on behalf of patients, in the patient’s electronic health record
6. Follow organizational guidelines regarding the use of the Electronic Medical Record (EMR) in compliance with HIPAA and patient confidentiality standards
7. Maintain access to the Health Information Exchange (HIN) and other related systems
8. Use HIN and other related systems to gather information needed to coordinate care and keep patients’ electronic health records up to date with the status of care that is being coordinated
9. Coordinate medical specialty referrals and procedures for patients going outside the patient centered medical home for care, as assigned, ensuring that referrals and procedures are timely, safe, effective, patient-centered, efficient and equitable
10. Process necessary prior authorizations and insurance referrals as assigned
11. Maintain surveillance ticklers and/or work with Health Information Technology to proactively identify the need for patient care, as assigned
12. Navigate patient to care, as assigned
13. Collect data related to healthcare coordination
14. Participate in committees, advocacy and outreach efforts as requested
15. Perform other duties as assigned
