In accordance with the Public Service Commission of Utah’s (the “Commission”) Rule R746-405, Central Utah Telephone, Inc. (“Central Utah”) submits for filing the original and five (5) copies of the Service Area Map/Exchange Boundaries and descriptions for the following Central UtahExchanges:

1. Fairview Exchange (427)

2. Fountain Green Exchange (445)

3. Scofield Exchange (448)

4. Mount Pleasant Exchange (462)

5. Soldier Summit Exchange (870)

6. Spanish Fork Canyon Exchange (873)

These Exchange Maps, as submitted, reflect the current boundaries of all Central Utah’s service areas, the principal streets, and the general location of the service area of the applicable Central Utah Exchanges in relation to nearby cities and major highways as required by R746-405-2(A)(4).

Central Utah hereby submits the enclosed service area maps pursuant to R-746-405-2(A)(4) and R746-405-2(E)(3) and requests that the Commission waive the 30-day filing requirement and authorize the updated Service Area Maps/Exchange Boundaries filed herewith to become effective January 1, 2014, for the reason that the revisions identify and define the existing service area boundaries for the applicable Central Utah Exchanges. Therefore, an immediate effective date will benefit subscribers and prospective subscribers in inquiries related to service area boundaries in the Central UtahExchanges.
