You are hereby notified that the undersigned terminates the rental agreement concerning the premises at:

Reason for termination of the rental agreement:
Effective the day of , , (date) and the premises are to be restored to the owner as of that date. Pre-paid rent and deposit, if any, will be dealt with in accordance with the Uniform Owner-Resident Relations Act and any agreement between the parties. Failure to vacate the premises by that date will result in a legal action being filed against you.
This is to notify you and you should be aware of the following:
(A) Regardless of any attempted correction, your Rental Agreement will terminate on the date specified above.
(B) The Secretary of Housing and Urban Development has required that within ten (10) days after service of this notice upon you, you may request and meet with the management of the property participating in a Federal Subsidized Housing Program to discuss the basis for this Notice. We will honor this HUD requirement. However, please be advised this notice will remain in effect as written unless otherwise withdrawn by written document.
(C) Office hours are from __________ a.m. to __________ p.m. on Monday through Friday. You or your counsel may visit the on site management office during office hours to view your tenant file, and copy any portions which are required for your defense.
(D) You have a right to defend this action in Court, if any court action is initiated. You may only be evicted by a court action.
