TENANT RELEASE AND CONSENT

I, ____________________________________________, authorized for you to release any information to KMS Enterprises, LLC without liability information regarding my current/past rental history and/or employment information.

INFORMATION COVERED:

I understand that previous or current information regarding me may be needed. Verifications and inquiries that may be requested include, but are not limited to personal identity; employment, income, and assets; rental history, medical or childcare allowances. I understand that this authorization cannot be used to obtain any information about me that is not pertinent to my eligibility for and continued participation as a qualified resident.

GROUP OR INDIVIDUALS THAT MAY BE ASKED:

The group or individual that may be asked to release the above information include, but are not limited to: Past and present employers, Welfare Agencies, Veterans Administration, Previous Landlords, State Unemployment Agencies, Retirement Systems, Public Housing Agencies, Social Security Administration, Banks and Others, Support Alimony Providers, Medical/Child Care Providers and Financial Institutions.

CONDITIONS:

I agree that a photocopy of this authorization may be used for the purposes stated above. The original of this authorization is on file and will stay on file for one year and one month from the date signed. I understand I have a right to review this file and correct any information that I can prove is incorrect.

Holding Fee:

In order to reserve this rental home, the Applicant will be required to pay at least half (½) of the monthly rent amount. Once approved, the Applicant will need to pay the remaining balance within two (2) weeks. If no Holding Fee is paid, KMS Enterprises, LLC will accept other applications and this home may not be available.

If the application is denied, the Holding Fee will be refunded to the Applicant.

If Applicant delays and/or cancels, the Holding Fee shall be forfeited.
