Hi, and welcome to the 2015-2016 academic year of the Health Science Careers Program at Rutgers, School of Health Related Professions and we hope you all had an enjoyable summer.

It is important to note that this year we will be combining our Advisory and Teachers meetings. Since we are now holding only two meeting this year, for the success of the program at you schools, it is essential that you commit to attending BOTH meetings.

The fall meeting will be held on Thursday, October 8, 2015 at 9:30 A.M and the Spring meeting will be held on Thursday, March 10, 2016. All meetings will be held on our Scotch Plains campus with a VTel connection to the Stratford campus. The meeting rooms will be posted at the entrance door for each meeting. Due to time restraints we are unable to send follow up meeting reminders however in case of a cancelation we will send notification electronically. Please check your e-mail the day before the meeting or before you leave your schools to download and print meeting materials as well as share with your colleagues.

This year’s Health Science Careers College fair is going to be held at the Liberty Science Center (http://www.lsc.org/) in Jersey City on Friday, December 4, 2015. It is shaping up to be a great day. Lots of colleges are planning to attend. You must make reservations by calling the center directly at 201-451-0006 ext. 1-310. The message will say “if you know your parties extension press 1” that is the 1 in the above extension.

· A PDF copy of your student roster currently in our database will be attached to this e-mail for your review. Please make necessary corrections on the roster. (If a student is missing from the attached database a new registration form must be completed and submitted). Once reviewed and corrected please create a PDF copy and send to Sherry electronically via (). If you prefer you may hand deliver to Sherry with other paperwork at the October 8, 2015 meeting.

· Registration forms (hard copy) for new student enrollment in the program are Due October 8, 2015.

· Grid Registration form. (Please submit electronically via email to Sherry Geardino at (). Please make sure your document is complete before you send. (Due October 8, 2015).

· Course Instructor Identification sheet (Remember; only those with current Rutgers, (SHRP) faculty appointment may teach the courses for college credit). (Due October 8, 2015).

· Clinical site list (Due October 8, 2015).

· HOSA list (Due October 8, 2015).

· Pat Olson Memorial Scholarship Recognition Criteria (Due March 10, 2016; for graduating seniors only).

· Clint Dozier academic excellence transcripts due (Newark and East Orange schools only) Students will be required to include a personal statement which includes career and educational goals. Transcripts must be included. (Due March 10, 2016; for graduating seniors only).

· Color-coded exam registration forms filled in, composite Roster Exam Registration Form with fees calculated, and Purchase Orders due by December 11, 2015 for January/February testing and April 15, 2016 for May/June testing).

Your students will not be allowed to test if paperwork is not received by assigned dates listed above. No fax or electronic versions will be accepted. (LPN instructors please see addendum)

· Grading system - This form should be REVIEWED with your parents and students and should signed by the parent. The form will be kept in your possession.

· Rutgers, SHRP Official Data Form which you will use to generate transcript requests for the upcoming 2016 submission as well as Course Status Forms (Due June 24, 2016; for graduating seniors only). hard copies only

· Student Transcript Request Form to be used as reference for you to review with grade 12 students.

· LPN calendar/schedule of events is enclosed. All LPN programs register for the semester closest to when you are taking exams i.e. if you are taking exam in July register for summer. (Please discuss with Suzanne before registering).

Tentative testing date assignments have been made and will be addressed at the October 2015 meeting. Sherry Geardino at ( will be handling exam scheduling).

Please take the time to complete all forms that are due October 8, 2015. All students MUST be registered and currently listed in our database to earn Rutgers, SHRP college credit.
