Re: Raffling Activities
The __________ Chapter of the Myasthenia Gravis Foundation of America, Inc. is requesting approval to conduct raffling activities on [insert date of one raffle] as part of our [e.g. Annual Meeting, Pancake Breakfast, Fundraiser, etc.]. [If multiple raffles are being planned in the future, list these out in the same format]
[For each raffle include the following two paragraphs:]
The __________ Chapter plans on conducting a raffle on [insert date] as part of our [e.g. Annual Meeting, Pancake Breakfast, Fundraiser, etc.]. The prize for this raffle is [list the prize; indicate cash, if appropriate], which was purchased from/donated by [list where the prize came from; if a cash prize, this is not necessary], and has a value of [indicate the market value of the prize]. Tickets will be sold [by mail/in person only] [in advance/on the day] of the drawing. EITHER - Tickets are sold at a cost of [price of the ticket] OR, if no consideration is necessary to participate - Although purchase of tickets is not required, the suggested donation is [price of suggested donation].
The total cost of conducting the raffle is expected to be [insert the total expected expenses associated with the raffle]. The raffle is expected to generate a net profit of [insert the net expected to come from the raffle] for the chapter, and no other individuals receive any portion of the proceeds [this assumes that no other individuals are receiving a portion of the proceeds – if someone is sharing in the proceeds, clearly indicate who they are].
EITHER – With regards to the above described raffling activities, no volunteer or paid staff member has spent or will spend 50% or more of their time over the six month period prior to the activity on developing or implementing the raffle activity.
OR—With regards to the above described raffling activities, [person(s) x (y, z)] has/have spent or will spend 50% or more of their time as a volunteer/as paid staff over the six month period prior to the activity on developing or implementing the raffle activity.
As required, enclosed are the following additional materials for our review:
1. Legal opinion from an attorney licensed to practice in the State of ______, verifying that raffling activities as planned by the chapter are legal within the state;
2. A resolution from the ____________ Chapter’s Board of Directors approving the planned raffling activities and verifying that the proper investigation has been conducted;
3. Copies of all promotional or publicity materials, including proposed press releases and mailing, to be used in connection with the raffling activities;
4. A copy of the raffle tickets to be utilized.
At this time, the ___________ Chapter requests written permission to conduct the above described raffling activities.
