Job Description
Job Title: Developmental Disabilities Medicaid Service Coordinator (MSC)
Reports To: MSC Supervisor Classification: Full-time
Grade: 8 Status: Non-exempt
Summary
Under the supervision of the MSC Supervisor, the Medicaid Service Coordinator (MSC) will provide individual case management services for developmentally disabled clients. The MSC is responsible for guiding and facilitating their clients access to necessary services designed to address specific client needs. The MSC promotes the concepts of choice, individualized services and supports, and consumer satisfaction.
Essential Job Functions
· Community outreach to identify eligible individuals and consumer enrollments.
· Develop, implement, review and maintain ISP’s based on interviews and assessment of consumer’s strengths and needs, working with both the consumer and their families.
· Provide service coordination, which includes information, outreach, linkage, referral, follow-up, monitoring, and advocacy to the developmentally disabled individuals and their families.
· Ensure that service plans are implemented and recorded in case notes and files.
· Coordinate services and resources for consumers.
· Submit accurate and timely monthly billing and reports to MSC Supervisor.
· Maintain caseload of 35 consumers with a maximum of 40 consumers, statistics monthly reports as required by program.
· Conduct home visits to ensure that consumer’s needs and compliance regulations are met.
· Assists in periodic case reviews as directed by Supervisor.
Knowledge, Skills and Abilities
· Working knowledge of the surrounding Rochester community including government agencies, non-profits, healthcare and other related service providers for PWDD.
· Technical knowledge of state and local agencies for provision of services for consumers.
· Possess knowledge of administrative processes, terminology and procedures.
· Proficient technical and computer skills necessary to accurately compile and process data and submit monthly/ annual reports.
· Excellent ability to communicate in written and verbal form with particular attention to detail.
· Ability to work professionally and compassionately with clients from diverse social and economic backgrounds and varying degrees of developmental disabilities.

While performing the duties of this job, the employee is frequently required to sit and talk or hear. The employee is frequently required to walk distances; use hands and fingers to handle, or operate computers, objects, tools, or controls; and reach with hands and arms. The employee must occasionally lift, and/or move up to 10 pounds and specific vision abilities required by this job include close vision and ability to adjust to distances and focus at various distances.
This position requires the ability to drive short distances on a regular basis getting in and out of a car. Position may require bending, stooping and walking; work is both indoors and outdoors and may have exposure to various weather conditions.
Qualifications
Preferred: A four year degree in the field of health or human services and no less than one year experience working with the developmentally disabled population or service coordination with any population.
Acceptable: A two year degree in health or human services field with no less than 1 year of successful work experience in direct service with developmentally disabled population.
Must be a NYS licensed driver with own transportation. A criminal background clearance by OPWDD is a requirement for this position. Bi-lingual Spanish a big plus.
