SCHOLARSHIP APPLICATION
INSTRUCTIONS: To determine your eligibility for each KHPR Scholarship, review each of the following:

Complete this form and use it as the cover page for your KHPR Scholarship submission packet. Submit your complete scholarship application packet by 5:00 p.m. CST on Friday, March 10, 2017 to:

Only complete applications should be submitted: Include all requested information for each scholarship in this packet INCLUDING the three reference forms each in a sealed envelope with the signature of the referee on the back across the sealed envelope flap.
APPLICATIONS THAT ARE INCOMPLETE WHEN SUBMITTED OR NOT TYPED WILL BE REJECTED.
In evaluating applications, the Scholarship Committee will consider the GPA, résumé, references (SCARFs), writing ability, and the record of, and potential for, contribution to the particular scholarship's field. Preference will be given by committee members to applicants who have made contributions firstly to their program in the Department of KHPR and secondly to the profession and KHPR in general, as indicated in the SCARFs, explained in the essay and listed in the resume. Please be aware of the individual score value of each application component
1. TRANSCRIPT(S) (Application score value GPA = 30%): Include your unofficial transcript(s) of all college course work from all universities/colleges
2. REFERENCE FORMS (Application score value = 60%): Arrange to have exactly three SCARFs (Scholarship Committee Applicant's Reference Form) completed by individuals who are familiar with the following: 1) your academic performance, 2) work or professional experiences, and 3) character. The combination of all three SCARF references must address the three areas listed above. The primary consideration for most department scholarships is academic performance and contribution to the department; therefore, it is highly recommended that your SCARFS be from faculty (UNT KHPR, other UNT faculty, or faculty from other Higher Education Institutions).
SCARFS MUST BE CURRENT AND HAVE BEEN COMPLETED WITHIN SIX MONTHS OF THE APPLICATION DEADLINE.
3. ESSAY and RÉSUMÉ (Application score value 10%): Essay: Submit a typed essay describing your career goals, and your academic and/or service contributions to the department and/or field (be specific in what you did and for how long), which may include leadership, volunteer experience, and professional development opportunities you have experienced. Individuals applying for specific scholarships are advised to write their essay to explain their longterm career goals and how those fit the scholarship award. Some scholarships require an additional essay to be eligible for the award. Check the Specific Scholarship Requirements for possible additional information to be included. Format: One page, size 12 font, single- or double-spaced, 1inch margins. Failure to follow the format guidelines will result in rejection of the application. RÉSUMÉ: Include an updated version of your resume.
