Reporting Requirements: By accepting Bellevue Schools Foundation funds, you agree to acknowledge funding, submit a Project Report and, if requested, provide a personal thank you to a donor or presentation summary describing the outcome of your project. This presentation will be in a format (tri-fold display, Power Point, etc.) appropriate for Bellevue Schools Foundation use. The Project Report is due upon completion of your project but no later than June 12, 2015.

Project Proposal

1.  Project Description. Describe your project, including how students will be involved and how the project enhances their educational experience.

2.  Student Outcomes. List and describe specific student skills, abilities, or behaviors expected to result from this project.

3.  Describe in detail any pre- and post-activities. Include how you intend to prepare your students and how you will follow up.

4.  Describe how you will evaluate the success of the project and the assessment criteria to be used.

5.  Project Timeline. Include a brief timeline for this project and anticipated completion date.

6.  List at least two ways in which you intend to acknowledge Bellevue Schools Foundation funding for this project.

Project Budget

Include itemized costs for this project in the table provided. If using outside resources, attach cost documentation (program brochure and cost, equipment price list, etc.). Be sure to include the name of businesses that will be contracted. Include expenses necessary for the project even if you do not intend to have Bellevue Schools Foundation pay for them.

If your project includes transportation costs, you must include documentation for this cost. For projects that plan to use District transportation (school bus), use the BSD Transportation Field Trip Estimator. An updated BSD Field Trip Estimator will be available in September.
