Please assemble in the order listed and submit to the CAS Dean’s Office through the Department Chair no later than October 1st of the year prior to the requested sabbatical leave.

1) Supporting the request
2) Addressing course coverage (note that approved sabbaticals must be
load absorbed.) Please include the course numbers and titles for the courses the faculty member would have taught during the sabbatical period and how those seats will be redistributed while the faculty member is on leave. For example, if the faculty member was scheduled to teach one 400-level elective with an enrollment capacity of 25 and one graduate level course with a capacity of 15, how will these 40 seats be made up?
Sabbatical Request Declaration Form (also signed by faculty member and
Department Chair)
Statement identifying any proposed income other than salary from the campus while on leave.
Statement acknowledging an obligation to return to the University at Albany for a minimum of one year at the conclusion of the leave.
Memo from faculty member requesting the sabbatical and a one or two-page
statement of the activities to be conducted during the leave period, including a description of the outcomes of the proposed activities as well as their importance to the University's educational program and the applicant's professional development.
Copy of the applicant's current curriculum vitae.
Supporting materials – e.g., copies of invitations from host institutions for a
period of residency or other letters of support.
If applicable, copy of the report following the applicant's last sabbatical leave
and/or other statements describing the value of the last leave.
