This memo is to request that you write a letter of recommendation on my behalf to be scanned in to my ERAS (Electronic Residency Application) for internship and residency.

You can help support my residency application by preparing and signing a letter of recommendation, saving it as a .pdf file and uploading it into the ERAS portal when it opens in Mid-May 2015. The following format would be ideal:

1. Use Letterhead to write your letter.

2. Address the letter to “Dear Program Director

3. Include a subject line with my full name.

4. Please include in your letter whether or not I have waived my right to see this recommendation as indicated below. IMPORTANT – IF THIS LETTER IS WAIVED, THE STUDENT IS NOT PERMITTED TO VIEW IT AND IT MUST BE KEPT CONFIDENTIAL.

5. Print, sign, scan and upload the letter to ERAS. (The ERAS portal opens in Mid-May, at which time you will be able to log into the following site with your AAMC account OR create a new account by clicking “Register for an AAMC account” and following the steps.) https://services.aamc.org/30/ssoLogin/home/login/login?goto=https://services.aamc.org/eras/lorportal/

I acknowledge that this letter is for the specific purpose of supporting my application for an internship or residency. I authorize the school to include any of my educational records in the disclosures made in the letter of recommendation.
