REGISTRATION FORM GUIDELINES

2011/2012 SEASON

All junior players are to register with their clubs using the current forms supplied. Junior coaches/managers must note the following:-

§ All details to be filled in. Players not deemed registered if all details not provided.

§ Ensure that forms are filled in legibly and all details are readable

§ Players register for an age group. Make sure that the right forms are used and any changes initialed by parents.

§ Players are to register for each age group that they play in

§ Completed registration forms are to be deposited in the box at QEO house

§ Players can not play unless registration form received prior to playing round

§ Progress lists of registered players are available on the web site and coaches/club administrators are required to check and ensure all players are registered.

§ Fax copies can be sent, but original copies must be forwarded as soon as possible
