Rate Calculation Exercises

Data for June

Unit / UTI / CAUTI / BSI / CLABSI / PNU / VAC / IVAC / Poss VAP / Prob VAP / Patient Days
(Resident Days for LTC) / Foley Catheter Days / Central Line Days / Ventilator Days
2 West / 4 / 4 / 1 / 0 / N/A / 625 / 250
3 East / 4 / 1 / 2 / 1 / N/A / 536 / 120 / 40
MICU / 2 / 2 / 3 / 2 / N/A / 3 / 1 / 1 / 1 / 300 / 200 / 175 / 125
LTC / 3 / 1 / 0 / 0 / 2 / 0 / 3750 / 750 / 0 / 0

How do you calculate?

1.  The UTI rate for 2 West, 3 East, and MICU (3 rates) 2 West: 4/625 x 1000 = 6.4 UTIs/1000 patient days

3 East: 4/536 x 1000 = 7.5 UTIs/1000 patient days

MICU: 2/300 x 1000 = 6.7 UTIs/1000 patient days

2.  The CAUTI rates for these same units The Catheter DUR for each unit

2 West: 4/250 x 1000 = 16 CAUTI/1000 catheter days DUR = 250/625 = .40

3 East: 1/120 x 1000 = 8.3 CAUTIs/1000 catheter days DUR = 120/536 = .22

MICU: 2/200 x 1000 = 10 CAUTIs/1000 catheter days DUR = 200/300 = .67

3.  The BSI rates for each of the three acute care units ßMay not appropriately measure the population at risk. Unusual to have

2 West: 1/625 x1000 = 1.6 BSIs/1000 patient days BSI that aren’t CLABSI nor secondary BSI

3 East: 2/536 x 1000 = 3.7 BSIs/1000 patient days

MICU: 3/300 x 1000 = 10 BSIs/1000 patient days

Data for June

Unit / UTI / CAUTI / BSI / CLABSI / PNU / VAP / Patient Days
(Resident Days for LTC) / Average Daily Census / Foley Catheter Days / Central Line Days / Ventilator Days
2 West / 4 / 4 / 1 / 0 / 2 / 0 / 625 / 250
3 East / 4 / 1 / 2 / 1 / 3 / 0 / 536 / 120 / 40

LTC / 3 / 1 / 0 / 0 / 2 / 0 / 3750 / 125 / 750 / 0 / 0

How do you calculate?

4.  The CLABSI rates for 3 East and MICU (2 rates) The central line DUR for each unit

3 East: 1/40 x 1000 = 25 CLABSIs/1000 central line days 40/536 = .07

MICU: 2/175 x 1000 = 11.4 CLABSIs/1000 central line days 175/300 = .58

5.  The pneumonia rate for the LTC unit

2/3750 x 1000 = 0.53/1000 resident days

6.  The 4 VAE rates for MICU The ventilator DUR for MICU

VAC: 3/125x1000=24/1000 ventilator days 125/300 = .42

IVAC: 1/125 x 1000 = 8/1000 ventilator days

Poss VAP: 1/125 x 1000 = 8/1000 ventilator days

Prob VAP: 1/125 x 1000 = 8/1000 ventilator days

7. The infection rate for the LTC facility using resident days

5/3750 x 1000 = 1.3 infections/1000 resident days Can also do CAUTI rate. May use average daily census for denominator.

Surgical Site Infection Rate Calculations

Dr. Smith / Dr. Jones / Dr. Adams
Number of colon surgeries performed / 66 / 65 / 82
Number of superficial incisional SSIs / 2 / 1 / 1
Number of deep incisional SSIs / 1 / 2 / 1
Number of organ/space SSIs / 0 / 0 / 1

Calculate the colon SSI rate for each surgeon.

Smith: 3/66 x 100 = 4.55%

Jones: 3/65 x 100 = 4.62%

Adams: 3/82 x 100 = 3.66%

If these are the only general surgeons for a facility, calculate the colon SSI rate for the facility

9/213 x 100 = 4.23%

What are the limitations of these SSI rates?

These are crude infection rates, with no risk adjustment. It means there can be no comparisons made over time, and no comparisons made other facilities. The only way to obtain risk-adjusted rates is to input required risk and outcome data into NHS, so that SIRs can be calculated.
