I believe the most challenging part of school is during the first week, when orientation requirements are being met. During this time, we review all policies and procedures with our students and establish guidelines for their success in our programs. In the case of high school students, a parent/guardian is also required to sign off on these forms, indicating approval. To simplify this process, I’ve developed a summary form to collect signatures, reducing the amount of paperwork to be returned.

After you have read and reviewed all related documents referenced, please sign below, indicating your acceptance and approval.

Photograph and Videotape Release

I, hereby grant to Francis Tuttle Technology Center and their representatives, the irrevocable and unrestricted rights to use and publish/broadcast photographs, video images, and audio recordings of me, or in which I may be included, for editorial, trade, advertising, and any other purpose and in any manner and medium; to alter the same without restrictions; and to copyright the same.

I hereby release Francis Tuttle Technology Center and their legal representatives, successors, and assigns from all claims and liability relating to said photographs, video images, and audio recordings. I understand that any use of my image will relate to the schools and its activities, and also that I will receive no compensation for the use of my image and/or name. I authorize the instructor or other classmates in charge of this process to post my picture on class web pages created (including Facebook, Twitter, or other Social Networking sites.)

Progress-based instruction

I understand policies related to individualized, progress-based instruction; if a student fails to utilize time properly, such as surfing the net, chatting, playing games, not using time wisely, additional hours may be added to his/her contract. Failure to maintain 70% progress and/or a 70% course average may result in removal from the program.

Student e-mail release form for parents/guardians of secondary students

I understand that as part of the Family Educational Rights and Privacy Act (FERPA) student records, including grades, are protected and may not be sent to me via e-mail. I further understand that FERPA has put this requirement in place because there is no guarantee of confidentiality on the Internet. My signature below will signify my understanding that e-mail is considered an unsecured electronic form of communication (i.e. person or persons other than the student may see or intercept e-mail communication). By providing my e-mail address and signing this release, I am granting permission for Francis Tuttle Technology Center to send my grades and other communication concerning my school records and account via e-mail, if requested. This may include: course grade reports, attendance status, account balances, financial aid status and other student information. Any electronic transmission of grades is not considered an official transcript. I further understand that this authorization will continue until it is revoked in writing and such revocation is delivered to my instructor. Internet Use Policy/FTP Access/Publishing Projects

Student Access

Students may compose and file transfer protocol (FTP) their own web pages that are directly related to their school projects or portfolio to the Francis Tuttle web server(s) and/or publish projects to a public portfolio. In addition to the instructor, potential employers and/or parents may also wish to review student work. This content will serve as the first steps towards a digital portfolio, which is vital to getting a job in this area.

Student web pages should:

¨ Contain their original work: content, writing, artwork, photography (etc.) should all be constructed by the student

¨ Be identified with the following text “This site was created for educational purposes.”

¨ Avoid links to game and entertainment sites on the World Wide Web

¨ High School students should avoid using personal contact information (address, telephone, school address, work address, etc.)

¨ Not contain any objectionable material or link directly or indirectly to such objectionable material (this is subject to the discretion of the instructor and administrators)

¨ Sites should not contain creepy or daunting material (such as skulls, skeletons, devils, etc.)

¨ Not use copyrighted materials unless prior approval from instructor is given and credits for copyrighted work(s) are cited.

Limitation of Liability

Francis Tuttle makes no guarantee that the functions of the services provided by or through the Francis Tuttle system will be error-free or without defect. Francis Tuttle will not be responsible for any damage the student may suffer, including but not limited to, loss of data or interruptions of service.

Disciplinary Actions

Routine maintenance and monitoring of the server may lead to discovery that a student has violated this policy, the handbook, or the law. Francis Tuttle will cooperate fully with local, state or federal officials in any investigation related to any illegal activities conducted through the Internet. In the event there is a claim that a student has violated this policy, the student will be provided with a written notice of the suspected violation and an opportunity to present an explanation before the Director. If the student has violated this policy, the student will lose their right to host their web page on the Francis Tuttle web server(s) and could be removed from the program.

Social Networking

Periodically students may be asked to reflect upon, share, and evaluate what they are learning. Many employers review posts on social networking sites before hiring—students are encouraged to always maintain a professional online presence. When used properly, these tools can aid the student in self-promotion and marketing their skills. Tools that may be used for this purpose include the following:

WordPress - A blogging site; students may be asked to write a summary of what they have learned, provide an evaluation, reflect upon their growth, and/or discuss the monthly professional skills topic.

Facebook – Our class Facebook Page, 3D Animation and Visual FX, will be used to support our class site. Events, Reviews and User Discussions may be conducted on our Facebook page. Periodically, students will be instructed to write on the class wall, and sometimes provide a link to their projects. The instructor may use this communication to review work and update grades; students can also use this opportunity to learn from each other.

Twitter/Professional Forums – Students may be asked to post links to resources they have found related to class that they can share with others, links to their work, troubleshoot, etc.

Behance/YouTube/Flickr/Instagram/Pinterest/Basecamp – May be used as an online portfolio, to inspire creativity, and organize projects. Students may also be asked to post final projects, portfolio components, etc.

I understand my class may utilize a class site, Facebook, or other Social Networking site to communicate with students. I approve of this use and agree to follow the policies outlined under the Francis Tuttle Acceptable Use Agreement when communicating in any class related activities on the Internet.

Francis Tuttle Acceptable Use Agreement

Acceptable Use for Francis Tuttle Students/Customers – Use of the District's networks, computers, software, and the Internet (“Computer Facilities”) by Francis Tuttle students/customers must only be in support of educational objectives and/or educational research as deemed appropriate by the instructor and/or administration.

Unacceptable Use for Francis Tuttle Students/Customers – Unacceptable use of the Internet includes sending or receiving any material in violation of any federal, state, or local regulations. This includes, but is not limited to, copyrighted material, threatening material, obscene material, pornographic material, and/or material protected by trade secret. Students may not use the Internet to advertise products or promote political points of view. Deliberate attempts to vandalize, damage, disable or disrupt the property of the District, another individual, organization, or the network.

Francis Tuttle's Networks, Computers, Software, and Internet Use is a Privilege

Use of the District's networks, computers, software, and Internet access is considered a privilege, not a right. Inappropriate use may result in school disciplinary action from revocation of user privileges up to and including suspension or dismissal from Francis Tuttle.

The following activities involving use of Computer Facilities are expressly prohibited at Francis Tuttle

Illegal Activities Illegal activities are strictly forbidden.

Security You may not:

- Initiate unauthorized transmissions of District owned information.

- Use another person's User IDs or passwords.

- Use another person's files, system or data without permission

- Communicate any information concerning any password, identifying code, personal identification number or other confidential information without the permission of its owner or the controlling authority of the Computer Facility to which it belongs.

- Use computer programs to decode passwords or access control information.

- Attempt to circumvent or subvert system security measures.

- Execute any form of network monitoring which will intercept data not intended for the student.

- Create, modify, execute or retransmit any computer program or instructions intended to gain unauthorized access to, or make unauthorized use of, a Computer Facility, Software or Licensed Software.

- Use Francis Tuttle systems for personal gain, for example, by performing work for profit in a manner not authorized by the District. Students who have been identified as a security risk or having a history of generating computer-related problems may be denied access to the Internet.

Copyright Software provided by Francis Tuttle is provided for a student's personal use on a single computer at a time solely for the purpose of participating in a course of study. Title to the Software (including intellectual property rights in the Software) is not transferred to the student.

You may not:

- Transmit or receive copyrighted materials (software, music, movies, etc.) not legally obtained.

- Make unauthorized copies of copyrighted materials, such as licensed software, music, movies, etc.

- Remove any proprietary markings, or distribute, copy, modify, make derivative works of, adapt, translate, decompile, reverse engineer or provide anybody else with access to the Software.

E-Mail Sending and receiving e-mail messages can only be done when it relates to their plan of study or when students are engaged in academic study or research. E-Mail is not guaranteed to be private.

Appropriate Content The following activities involving use of Computer Facilities are prohibited:

You may not:

- Transmit information, which contains obscene, indecent, or lewd material or other material, which explicitly or implicitly refers to sexual conduct.

- Use mail or messaging services to harass, intimidate, or otherwise annoy another person, for example, by broadcasting unsolicited messages or sending unwanted mail.

Appropriate Language Swearing, the use of vulgarities, or other inappropriate language will not be tolerated.


Network Disruption Students must not use the Internet in a manner that is disruptive to others or that interferes with the operation of Francis Tuttle’s network or any other institution’s network. This includes, but is not limited to, uploading or creating computer viruses and any attempt to harm or destroy hardware and/or data of another user.

You may not:

- Effect security breaches or disruptions of network communication. Security breaches include, but are not limited

to, accessing data to which the employee/student is not an intended recipient or logging into a server or account that the employee/student is not expressly authorized to access. For purposes of this section, “disruption” includes, but is not limited to, network sniffing, ping floods, packet spoofing, denial of service, and forged routing information for malicious purposes.

- Engage in any activity that might be harmful to systems or to any information stored thereon, such as creating or propagating viruses, disrupting services, or damaging files.

- Create, modify, execute or retransmit any computer program or instructions intended to obscure the true identity of the sender of electronic mail or electronic messages, collectively referred to as "Messages," including, but not limited to, forgery of Messages and/or alteration of system and/or user data used to identify the sender of Messages;

- Initiate port scanning or security scanning without written permission from the Chief Technology Officer.

Francis Tuttle Acceptable Use Agreement Disclaimer

Francis Tuttle is not responsible for the accuracy or quality of information obtained through the Internet. Francis Tuttle takes precautions to eliminate the access of inappropriate Internet sites.

I understand that my usage of the Internet and/or computer may be logged, monitored and/or reviewed by Francis Tuttle personnel. I am responsible for using these resources according to classroom policies and procedures. I further understand that I will be subject to disciplinary action, up to and including suspension/dismissal for any violation.
