Please help us get to know your child better by answering the following questions. The information that you provide is critical in allowing us to best represent your son or daughter in the recommendation we write for admission, scholarships, and other programs both within our school and in the community. You may use the space provided, attach separate pages, or e-mail it to the address at the bottom of this document.

5. What does your child usually do during his/her spare time?

6. Describe an event, activity, or time when your child seemed especially excited about or involved in what he/she was doing.

7. Describe a recent time when your child had a profitable struggle, a courageous victory, or gained hard-won wisdom.


8. Are there any family circumstances that would be pertinent for me to know as a counselor or for a college admissions officer to know? (blended family, deceased parent, etc.) Are there any financial limitations that could impact where your student applies?

9. Describe any unusual personal characteristics or events that have affected your child's educational experiences or personal development (ex.: repeated or skipped grades, excessive absenteeism, ADHD, medical issues.)

10. Please share with us the colleges you think would be a good match for your student. Please tell us why you find them appealing for them in particular.

11. PARENT OPTION TO ANSWERING THE ABOVE QUESTIONS: Put yourself in our position as your child's college counselor and write a letter of recommendation to his/her college including elements of leadership, commitment, and talent that we might not be privy to. Anecdotes, unusual experiences, and humorous stories are especially helpful. Attach letter to this form or e-mail it to us.
