FIELD TRIP
PARENT PERMISSION
This letter gives my son/daughter permission to attend the West Forsyth Science Honor Society field trip to the North Carolina Zoo in Asheboro, NC on Saturday, May 17, 2014. I understand that my child will leave West Forsyth via activity bus at 9:00AM and return no later than 4:00PM.
I also understand that this is a voluntary event, and SHS members that intend to participate are required to turn in this completed permission form and $11 to reserve their spot by
Friday, April 11, 2014.

· Mark your calendar now with the field trip date.
· Turn in the field trip permission form and $11 by Friday, April 11, 2014.
· On May 17, meet in the student parking lot at 8:45AM. Bring a bag lunch with a drink or money to buy lunch at the park. You may also want to bring additional money for the gift shop.
