Online Evaluation & Credit Claim Form

The Case Western Reserve University Continuing Medical Education Program is now offering online evaluations and credit claim form tools to activity directors. To take advantage of these new tools, you will need to send a copy of your brochure outlining the speakers for the course along with the number of credits available for the course to Karen Popil, the CME Registrar.

In the hard copy of your materials as well as any information you email to the participant, you will need to include the following verbiage:

Thank you for participating in the ____________________ course. To claim credit for the course and receive a printable certificate, please go to http://cmetracker.net/CASECME/getCertificate.html. If you registered online or have used the Case Western Reserve University CME website in the past, enter your email address, click on “I have a password” option and enter your password and CME Code. If you registered via phone, fax or mail and have never logged into the CWRU CME website, use the email address you supplied for registration and select the “I am a new user” radio button. At this time, you will NOT be able to enter your code. You will first be prompted to create a profile. Once your profile is created, you will then be taken back to the original log in screen and will follow the instructions above by entering your email and clicking on “I have a password”.

You will be taken to an online credit claim form. You should only claim credit commensurate with your participation in the activity. Once you submit your credit, a certificate will come up that you can print for your records.

Please keep your email address and password that you create in a safe place. This is how you can register for courses, download a free transcript, and continue to claim credit for future activities.

The link will always remain the same along with the verbiage. The only information that will change is the CME Code that you give to participants. You will have a different CME Code for every activity you do with us, so please make sure you check with the CME Registrar, Karen Popil, before giving out the information.
