No entry will be accepted without signature, necessary proof of qualifying information, and entry fee. All competitors must have taken and passed the 5-year polygraph or urinalysis drug test given by the SNBF officials. Proof of age for Teens and Masters must be presented at check-in. This is a SNBF contest and will be run in accordance with the rules and procedures established by the SNBF officials. In consideration of your acceptance of this entry form and fee, I waive and release all rights and claims for damage against Supernatural Bodybuilding/Fitness Contest, promoters, sponsors, and their associated bodies, officials, agents, employees or assigns by the reason of injury or damage which I may incur while traveling to or from, or participating in this competition. I further agree to abide by all rules and decisions made by the SNBF committee. I declare that all information contained herein is, to be best of my knowledge, true, correct, and complete. I agree to hold harmless waive, and release all photo and video rights for magazines, newspapers, television, video distribution, and other media.

Evening - 6:00 p.m. All Competitors Check-in at 5:00 pm. 60 second Bodybuilders routine. No Profanity/Vulgar music allowed. Group comparisonsfor everyone else - EVERYONE will pose during Finals unless changed by promoter. Class adjustments may be made by promoter if needed.
