Dear Parents of Monroe Township Music students:
On Friday, June 5th, the Monroe Township music program will travel to Fleetwood school district and Dorney Park to perform and listen to other ensembles in the Music in the Parks Festival. Due to the large number of students involved in this expedition, we will need a $50 deposit made payable to MTMS returned to us by January 15th along with the permission slip to secure an accurate number of tickets. Every member of the ensemble is needed to perform up to our expectations. Students must maintain a grade of 88 or better in band/chorus to be eligible for participation. Please review the following information regarding the trip and feel free to contact the school with any questions or concerns.

7:30 A.M. – 10:00 A.M. Bus 1 and 2 travel to Kutztown High School(50 Trexler Avenue, Kutztown, PA 19530)
10:20 A.M. – 10:40 A.M. Chorus warm up
10:40 A.M. – 11:00 A.M. Choir performance
11:00 A.M. – 11:20 A.M. Chamber Singers warm up
11:20 A.M. – 11:40 A.M. Chamber Singer performance
11:45 A.M. – 12:30 P.M. Choir lunch, change and reload busses
12:30 P.M. – 1:00 P.M. Bus 1 and 2 travel to Dorney Park_______________________________________

11:45 P.M. – 12:10 P.M. Falcon Jazz Band warm up; other band members lunch
12:10 P.M. – 12:35 P.M. Falcon Jazz Band perform; other band members lunch/change
12:35 P.M. – 1:45 P.M. Jazz Band members eat lunch; other band members reload busses
12:45 P.M. – 1:00 P.M. Bus 3, 4 and 5 travel to Dorney Park; Jazz Band members change___________________
1:00 P.M. – 1:15 P.M. Meet chaperone at entrance to Dorney Park, distribute tickets, locate meeting point
3:45 P.M. – 4:00 P.M. Meet at Dorney Park Arena; distribute dinner tickets
4:15 P.M. – 5:00 P.M. Optional awards ceremony in the park/Dinner
5:00 P.M. – 7:15 P.M. Explore the park
7:15 P.M. – 7:30 P.M. Meet at location point; reload busses
7:30 P.M. – 10:00 P.M. Travel back to Monroe Township
10:00 P.M. – 10:15 P.M. Unload and board buses and have a great weekend
TRANSPORTATION AND SUPERVISION
Round trip transportation will be via coach buses. In addition, approximately one teacher will accompany every ten students. The trip cost will be around $110. The actual total will be determined once an accurate number of participating students is found. Students can bring money for snacks and/or souvenirs, backpacks, iPods or MP3 players. These items must be left on the busses during the performance. School officials cannot guarantee the security of items kept on the bus.

2. I understand that this form must be completed and turned in to my childs’s homeroom teacher by the designated deadline.

If your child has a life threatening medical condition, (i.e. asthma, anaphylactic reaction) which requires medication, please contact the school nurse for a request for self-administration of medication form which your doctor must complete and sign. The completed form must be returned to the school nurse prior to the date of the field trip. Medication must be in the original prescription labeled container. For any questions regarding medication on field trips, please contact the school nurse at 732-521-6042 and press “2” to reach the nurse’s office.
I have read the contents of this form and hereby give permission for my son/daughter to attend the field trip. I also understand that school officials have the right to conduct reasonable searches of student backpacks or other belongings.
As per Board of Education Substance Abuse Policy #5330, and in accordance with N.J.S.A.18A:40A-12, if I cannot be reached, I give the district my permission to seek emergency medical treatment for my child and be tested for controlled dangerous substance if there is reasonable suspicion reported.
