Module Entry – Exceptional Amendment Form

·  Students are able to change their optional module choices during the two week ‘change of mind’ periods at the start of each semester with the approval of the home School of the student and the offering School of the module (where different).

·  After these ‘change of mind’ periods, changes to module registration will only be accepted in exceptional circumstances and with the support of the student’s home School. This form should be used to request such changes.

·  Any request to change a module registration once the majority of teaching for the module has been delivered (in the case of year long modules this will apply when the majority of teaching in the initial semester has been delivered) must be approved by Quality and Standards Committee and will only be possible where there are extenuating circumstances.

These module entry changes are suitable for my course, have been approved by the School/Department running the modules I will be taking and by my home School. I understand that students will not be allowed to drop modules when the majority of the teaching of the module has been delivered. This would only be possible when there are extenuating circumstances.

·  If the majority of teaching for the module has not been delivered please amend the enrolments on the student’s record or return the form to Curriculum Management, Academic Services Division for input. Please also notify the Exams Office, Academic Services Division, so that any examination arrangements that may be in place can be adjusted.

·  If the majority of teaching for the module has been undertaken the School will need to make a case to Quality & Standards Committee for approval not withstanding regulations. Please use the space below to explain the student’s extenuating circumstances and why the School supports the change. Please return the form to Student Administration, Academic Services Division, who will liaise with QSC and notify you of the outcome.
