For further information regarding development and initiation of local health, mental health, vision or dental care partnerships, please contact your Local District Organization Facilitator.

The increasing interest in the provision of school-based health activities and District decentralization efforts has contributed to a need to revisit and strengthen the process of developing health service agreements.

The basic information regarding volunteer personal and professional health services for physical health, mental health, and vision services is provided in Office of the Deputy Superintendent, Bulletin No. 40 (Rev.), dated June 15. 1994; and Student Health and Human Services, Bulletin No. Z-56 dated June 30, 1998. The revised agency applications, operating guidelines, and template Memorandum of Understanding to establish these partnerships are now available through your Local District Organization Facilitator. Critical elements of this process include:

· an agency application to provide volunteer services;

· a Memorandum of Understanding (MOU) that is reviewed by the Directors of Student Medical Services, Nursing Services, Integrated Student Health Partnerships, and Mental Health as appropriate;

· documentation of licensure, TB clearance and current certificates of insurance naming the Los Angeles Unified School District as co-insured;

· a school site delivery plan; and

· a complete packet is returned to the Local District Organization Facilitator who will finalize the application process.

School-based health clinics offering medical diagnosis and treatment services require an LAUSD approved School Based Health Clinic Operating Agreement. Volunteer services should not initiate until the lead school's administrator has received notification that an MOU or a School-Based Health Clinic Operating Agreement has been approved indicating that the partnership application process has been successfully completed.
