Checklist: Laboratory – Specific Biosafety Training by Principal Investigator

To each Principal Investigator:

Unplanned exposures and laboratory-acquired infections are preventable among staff handling biohazardous agents. The information provided in the program “Biosafety in the Research Laboratory” provides the basis for more specific training in each laboratory. Please take the time to review each item on this Checklist with each member of the laboratory staff working with biohazardous agents.

NOTE: Please retain this document for each staff member for your records. During annual inspections, please make these documents available for the inspecting biosafety officer.

The PI or his/her designee has reviewed all the biosafety registrations pertinent to the laboratory that may impact me when I work in this laboratory.

The PI or his/her designee has reviewed in detail the aspects of the biosafety registration(s) directly pertinent to my project(s), including appropriate engineering controls, protective equipment, proper inactivation and/or disposal of biohazardous/medical waste, and spill clean-up.

The PI or his/her designee has given me a copy (or copies) of the biosafety registration directly pertinent to my project(s).

The PI or his/her designee has given me copies of the relevant exposure response plans for this laboratory.

The PI or his/her designee has verified that I am adequately trained to perform the project(s) I am undertaking, or has assigned my training to the following individual:
