Introduction

Although various attempts have been made, and will continue to be made, to rationalise the provision of neonatal beds there will always continue to be a need to arrange Intrauterine and Neonatal Transfers at short notice.

And although various attempts have been made to facilitate such transfers through the creation of regional bed bureaus, it is the writer’s personal experience from working in several different regions that such bureaus do not work. Nothing substitutes, or is likely in the future to substitute, for the tedious process of telephoning directly - too often in the middle of the night - first to each neonatal care unit in turn to find a bed, then, having found a neonatal bed, to the labour ward to confirm that they are able to accept an intra-uterine transfer, and finally, via the switchboard, to talk personally if necessary, to the appropriate consultant. Such a procedure, if it has to be done via the originator’s switchboard and then the potential destination switchboard, wastes an inordinate amount of time which could be better spent. As a result the writer has, as a consultant locum in at least four different regions, created a laminated local list of all the relevant direct lines.

Having done all this tedious work, and at the same time having created the Electronic Encyclopaedia of Perinatal Data, it seemed worthwhile making the results available to others via the EEPD.

Local Laminated List

It is suggested that, in time, each birthing unit and neonatal department should create a laminated list, sorted by distance, and by the level of neonatal care, to be kept by the appropriate telephones. A sample of such a list is given under http://www.fawdry.info/eepd/g_add/SampleG.doc and the top few lines of a blank MSWord page for local completion are given below.

In time it is intended that each hospital in the full list below should have a postcode attached so that local mileages can be easily ascertained using Google Maps.

Levels of Neonatal Care

Level 3 Neonatal Care Units (L3)

According to the British Association of Perinatal Medicine (as cut and pasted in July 2008 from a document on the BAPM website) the definition of a Level 3 Neonatal Unit is as follows:

“Where continuing neonatal intensive care is provided (a Level 3 Unit), the Unit should be staffed by consultants whose principal duties are to the NICU. All new appointees to such posts should have CCST in Paediatrics (Neonatal Medicine).

Where continuing neonatal intensive care is provided (Level 3 Unit), there must be 24-hour resident cover by a doctor who has completed General Professional Training and in addition has experience equivalent to at least one year of ‘core’ Higher Specialist Training in paediatrics, including four months of neonatology. This doctor should be available for the Intensive Care Unit at all times and not be required to cover any other Service.”
