INSTRUCTIONS
1. Juniors who have participated in at least one sport and one non-sport IHSAA sponsored activity are eligible.
2. Application must be typewritten. Use only space provided; addendums will not be considered.
3. School administrator, teacher or counselor must verify items #1-3 and complete item #4.
4. Return application to IHSAA by November 10 (fall selection) or April 10 (spring selection).
