1)  Mark with an X the box under “LST” to indicate your intention to take the course as credit. If you intend to take the course as audit, write “audit”. A maximum of 4 courses (12 units) may be credited each semester. In case of overload, request permission from the VP for Academic Affairs.

2)  In courses with 2 sections, please do not indicate the choice of section and professor. Sectioning will be done during online enlistment at isis.lst.edu. If the quota for a section has been reached, students will be automatically enlisted in the other section.

3)  Students enrolling in pastoral counseling (CEFAM) courses should secure an Advisement Slip issued by the CEFAM President or his delegate.
