Important Information About Returning Your Animas® Insulin Pump

Enclosed is your replacement Animas® insulin pump and prepaid return shipping label. Please use the box in which you received your replacement pump to return your current pump. You will be instructed to affix the enclosed shipping label to this box in order to return your products. Should you need assistance in programming the replacement product, please contact Customer Support at 1-877-937-7867.

If you have been using long-acting insulin while waiting for your replacement pump or if you have any questions about insulin dosing, contact your healthcare provider before using your replacement pump.

Your original shipment included a User’s Guide. If you would like to view a copy of the User’s Guide, please visit our website at www.animascorp.com.

Please copy the following three pieces of information from the shipping label to this form and retain it for your records. Do not place the form back in the box. (See reverse side for sample labels)

Step 1. Verify that the serial number on the shipping label matches the serial number on the back of the pump being returned. If it does not match, please call Animas Customer Support at 1-877-937-7867 between Monday through Friday 9 am - 5 pm Eastern Time.

Step 2. Remove the battery and any accessories you use when wearing the pump.

Step 3. Place the pump you are returning into the protective plastic case in which your replacement pump arrived, then place this pump into the cutout of the box. Please make sure that you return the battery cap and cartridge cap along with this pump.

Step 4. If you were instructed to return additional items, place them inside the box.

Step 5. Place the return shipping label over the current shipping label on the box and securely tape the box closed.

Step 6. Check the shipping label for the name of the return shipping company. Please check the appropriate company’s website or use the phone numbers listed below to locate the nearest drop box.

Step 7. You can verify that the package has been returned by entering the return tracking number in the search field of the associated shipping company’s website.
