How do I file a Disclosure Form?
Access the Acknowledgement and Disclosure Website:
The URL for the website is: https://www.maricopa.edu/disclosure
It may be helpful to review the Frequently Asked Questions before submitting your acknowledgement.
Select the Acknowledgement you are submitting.
You will be prompted for your User Name and Password.
Your User Name is your Maricopa email address: (e.g. -- )
Your password is your email account password
Complete the questions that are asked as part of the survey. Be sure to answer all of the questions by clicking Next after each question.
On the last screen click the Finish button to complete your response.
