I also understand that any questions I may have regarding this information should be directed to the building principal. The handbook is available to view at getsd.org under the middle school tab or a hard copy may be picked up in the middle school office.

As the parent/guardian of ___________________, I have read the Student Safe and Responsible Use Policy for Digital Resources for the Gale-Ettrick-Trempealeau School District. I understand that this access is designed for educational purposes. I also recognize it is impossible for the Gale-Ettrick-Trempealeau School Board to restrict access to all controversial materials and I will not hold the Board or its employees responsible for materials acquired on the network/Internet. I understand that any district owned digital resource may be monitored at any time.

Photographs or student names may be posted to the district website if this form is completed granting this permission. Student names will not be posted with pictures.

I have received this policy and am aware that my child may be accessing the Internet.

Please read the Student Safe and Responsible Use Policy for Digital Resources carefully before signing this document. This document must be signed before you will be given school access to the internet and other digital resources.

I will not type any form of profanity or inappropriate, rude or suggestive language.

I will not view any inappropriate sites.

I will not download any inappropriate materials.

I understand that any district owned digital resource may be monitored at any time.

The Gale-Ettrick-Trempealeau School District School Board has approved the attached Student Safe and Responsible Use Policy for Digital Resources. If any user violates any of these provisions, access to the internet and other digital resources will be terminated and all future access could possibly be denied.

I have read the rules and agree to the conditions of the Student Safe and Responsible Use Policy for Digital Resources for using the internet and other digital resources. I understand that my digital resource privileges will be suspended if the rules are not strictly followed.

Gale-Ettrick-Trempealeau School District iPad Self Insurance Information

The Gale-Ettrick-Trempealeau School District is offering an insurance policy on student iPads that covers accidental damage inside or outside of school. A payment of $30 (per device per year) will cover any non-warrantied coverage that may occur during the 2016-2017 school year. The maximum fee any family will pay is $60 per year.

Your child is agreeing to keep the iPad in the supplied case. The policy provides coverage against accidental damage. Broken screens cost about $120 to repair. Students are also responsible for cords and chargers, which cost $10 each to replace.

Examples of accidental damage include, but are not limited to drops, falls, collisions, and liquid spills. The item is covered on/off school property, at home or traveling. We DO NOT cover theft, loss or intentional/negligent damage.

Coverage for Multiple Incidents

Accidents happen, so we will apply the following deductible beyond the first repair:

What does the self-insurance plan cover?

● Drops - From cracked screens to broken internal parts.

● Spills - Doesn't matter if it's one spill or full immersion in a swimming pool—we've got you covered.

● Broken Lightning Port - If you need to wiggle the power cord or hold the cable in place to get a charge, it might be the port, not the cable. If so, we've got you covered.

● Touch screen Failure - We cover broken touch screen digitizers, which can result in a display that looks active but doesn’t respond to touch commands.

● Display Failure - We cover dead pixels, white lines, or total blackouts that can make an LCD screen unusable.
