POOL MODIFICATION PERMIT PROCEDURES

1. Complete all pool and contractor data and sign permit application.

2. Enter all appropriate data on the Modification Checklist and sign at the bottom.

3. Submit necessary manufacturer=s specification sheets, plans, samples, and any other

supportive data with the application.

4. Submit $75.00 permit fee by company check or money order made out to the Fulton County

Department of Health and Wellness. Cash will not be accepted.

5. All permits will be issued by the Environmental Specialist. Direct all applications, supportive

data and inquiries to the Environmental Specialist. If you want to apply for a permit in the

office, contact the Environmental Specialist to make an appointment.

6. A modification permit is required for each equipment change. A major modification will

require permitting. A minor modification will normally not require a permit. Contact the

Environmental Specialist prior to any work performed to determine if it is a major or minor

modification and if you need a permit.

7. Do not start any major modification without a permit or any minor modification

without permission from the Environmental Specialist.

8. A separate application and fee are required for each pool.
