Instructions

For J-1 Visa Holders Traveling Outside the United States:

In order to travel outside the United States and return to your program activities, your DS 2019 must be validated for travel. Validations are effective for one year from the date of signature or until the end of the program, whichever is sooner.

Government Visitors, Specialists, Professors, Research Scholars & Short-Term Scholars

1.  Request appropriate leave from unit host.

2.  Complete, sign and date this request form.

3.  Submit Form to International Programs (IP) with original DS-2019 and copies of passport picture pages, visa pages and entry stamps, and copy of the I-94 card.

4.  Visit with International Visitor Program Coordinator in International Programs to discuss your travel plans. In this way, any potential complications with your immigration status and travel can be addressed.*

5.  Pay all local accounts in full before departing.

*Please inform the International Visitor Coordinator if:

-you plan to be gone from the US more than 30 days

-your J-2 dependents will be staying in the USA while you are gone

-you are supported by a grant or assistantship

-your DS-2019 is issued by another sponsoring agency

(in such cases, this form must still be completed and submitted to International Programs to maintain records of your travel. International Programs will assist in sending your DS-2019 to your sponsor for validation, if necessary.)

Send or bring the following items to the International Programs Office:

□  Original DS 2019s for all J-1 and J-2 visa holders planning to travel

□  Copy of valid I-94 card

□  Copy of most recent US entry and exit stamps in passport

□  Complete Travel Validation Request Form
