NOTE: This checklist addresses only the sections of the Americans With Disabilities Act Accessibility Guidelines (ADAAG) that are common to most DEO divisions or programs. Guidelines under ADAAG that address fire, safety, and building codes are included on a limited basis.
Different standards may apply to different buildings depending on the date the facility was built or last remodeled. The only exception is for “readily achievable” standards which are required for both old and new buildings. Please be sure to accurately state the date the building was built or last remodeled so that the correct standards can be applied. The section below should be completed in its entirety.

GENERAL INSTRUCTIONS: A separate copy of this checklist should be completed for each building where employees work or services are rendered to the public. The person or persons completing the accessibility survey should feel free to duplicate and use these materials as necessary.

Most items should be marked "Yes," "No," or "N/a." If an entire section of the checklist is not applicable (for example, Section K: Elevators or Section L: Stairs), check the column “N/a” at the beginning of the section: no other entry is required.

If, for lack of proper testing equipment, a listed requirement is not checked (for example, the decibel level of an elevator signal), indicate “Not checked - technical reasons” in the Comments or Notes section. Any item left blank will be considered to be NOT in compliance. Therefore, you should comment on all "no" or "n/a" responses. We want to know the details of your special circumstances.

New Construction, Additions and Alterations: Certain items in the checklist are annotated as being applicable to new construction. Depending upon the additions or alterations that are planned for an existing facility, requirements that are applicable to new construction may also be applicable to the planned additions or alterations. It is incumbent upon the facility manager or lease holder to ensure that accessibility guidelines are being properly addressed.

Measuring Slope: Several items in the checklist refer to a maximum permissible slope, expressed as a ratio (for example, a maximum slope of 1:12). A maximum slope of 1:12 means that there can be no more than a 1-inch vertical rise, measured perpendicularly, for every 12 inches in length, measured horizontally.
