To be completed by the child's pediatrician.
q "A Parent's Guide to Regulated Child Care" Brochure - Please sign,
detach, and return only the signature page (you keep the brochure).
q “All About Me” Form – Required if child has special needs.
If not, it is optional.
q Medication Form – if medication must be given by childcare staff
Completion of these forms is required by MSDE so your child may begin the program as planned. If you have any questions, check with the Site Director.
