FASHION SHOW GARMENT SUBMISSION FORM
If you are an EXHIBITOR, please use the form found in your Exhibit Kit – thank you!
Form Deadline: June 30th via mail, fax or email or on-site by 4 p.m. July 14, 2016
Please use this form to enter a garment in the CGOA Member Fashion Show held Sat. evening July 16th. There is no fee for individual CGOA/TKGA members to participate. Limit: TWO garments or ONE ensemble.
Make two copies of each form (one garment per form – must be easily readable):
1) Submit one copy by Deadline. Fax: 740-452-2552. E-mail: .
Mail: CGOA Conference, 1100-H Brandywine Blvd, Zanesville OH 43701-7303 or turn in at the conference registration desk no later than 4 p.m. on Thursday, July 14th.
2) Attach the completed form to the garment. Turn in garment with form to the registration desk by 4 p.m. on Thursday, July 14th.
How to keep your garment from being rejected: Garments without forms or with unreadable forms will be rejected. Garment description must be complete. Garments without sewn in labels will be rejected (each garment must have a sewn in label identifying who to return it to). Forms and garments need to be turned in on time.
