This evaluation tool can be used for any visit during which the resident performs OMT. A compilation of these evaluations will be used in the resident semi-annual evaluation.
Developing / Competent for Level of Training / Exceeds Expectation1. Appropriately identified somatic dysfunction relating to chief complaint
2. Discussed differential diagnoses with staffing physician
3. Properly explained technique to patient and obtained verbal consent to perform
4. Properly performed technique appropriate to somatic dysfunction diagnosis
5. Provided appropriate patient instructions and follow-up care
6. Documented procedure appropriately in the EMR
7. Assessed patient comfort throughout procedure
