CHECKLIST FOR SPRING & FALL PROGRAMS

_____ Chaperone Guide – Provide to parents or other adults who will be joining you for your Bryant Pond visit. Select parents or other adult chaperones based on the

number of students you will have (10:1 ratio). We ask that schools provide additional support for children with behavioral, physical, or mental challenges. This does not necessarily need to be an employee of the school, but someone (possibly a parent) who can carry out a behavioral plan with the student if one is in place in the classroom. The adult may provide extra help in negotiating trails and bathrooms in the case of a physical challenge.

_____ School Information Sheet - Mail or fax to Lyndsey Smith at the above address as soon as possible. This will be our confirmation from you in terms of dates, times, numbers, goals, etc.

______ Name Tags for Students – While not necessary, it is often helpful to the instructors when students come with nametags on the day of the program.

______ Equal Opportunity for All Form - Bring completed form on the day of the program, please make sure it includes all students attending the program.

_____ Learning Group Planning Sheet - Contact the Lakeside Classroom Coordinator to

determine the number of learning groups that you should plan for. Please fax or mail this sheet to Lyndsey Smith prior to the program so she can provide them to Bryant Pond staff and make copies for chaperones.

_____ Bunkhouse Assignments- Having bunkhouse assignments saves time and trouble when the students are moving in for overnight programs. The bunkhouse has 8 rooms for girls and 8 rooms for boys which house 4 students/room. The adult area in the middle holds 4. There is an adult room on both the boys and girls side. The Lakeside Lodge has 4 large bunkrooms that hold 8 students in each room. There are adult rooms on each of the two floors which hold 2 adults. (Please contact Lyndsey with questions)

_____ Medical Authorization Form - Collect completed forms from all participants, including teachers and chaperones and review prior to the program. Alert the Lakeside Classroom Coordinator to any conditions or issues of which the Bryant Pond staff should be aware (including any special needs of teachers or parent chaperones) as far in advance as possible. Bring the forms with you on the day of the program along with all medications. PLEASE NOTE: The Bryant Pond’s Health Coordinator will dispense and administer prescription and over-the-counter medication. Parents should make arrangements to provide classroom teachers with all needed medication, supplies or instructions in their original containers.

______ Assumption of Risk Form - Collect completed form from each participants including teachers and chaperones and bring to Bryant Pond on the day of the program.

_____ Photo Release Agreement - Collect completed form from every participant and bring

on the day of the program.

_____ Equipment List - Review with students and send a copy home for parents. Call Lyndsey Smith if you have questions. Some students may not have rain gear and trash bags can be used as a substitute.

_____ Parents’ letter - Send a copy home to all parents (optional).

_____ Maps/directions to Bryant Pond-Call if you need specific questions.
