PATIENT EDUCATION AWARD

APPLICATION PROCESS

1. The patient and family learning need is identified.

2. Any multidisciplinary staff member may recognize the resource need and complete the proposal for educational funds. (see attached form)

3. The proposal is submitted to Hope Odzak, and may be dropped off at Room 7453 Duke North by June 2, 2008.

4. By June 16, 2008, the Heart Center Patient Education Council will prioritize and select proposals based on the identified selection criteria. (see attached form)

5. The Heart Center Patient Education Chairperson will present recommendations to the Heart Center Executive Committee for approval in July 2008.

6. The Heart Center Development Officer will communicate approval of funds and appropriate as per designated procedure by the Development Office.

7. The recipient of funds will submit a follow up evaluation of the project to the Heart Center Executive Committee within six months.

8. The amount of funds available annually will be determined by the Heart Center Executive Committee and Development Office and approved by the Heart Center Director.
