Coaches and Leadership Plenty facilitators,

It’s time to prepare for the upcoming Leadership Plenty sessions in your community. With that comes the paperwork the Northwest Area Foundation and our university require as part of the grant and research guidelines.

Included with this instruction sheet you will find the following:

·  Youth assent forms (for those under 18)

·  Adult consent forms (for those 18 and over)

·  Leadership Plenty pre-surveys

·  Leadership Plenty post-surveys

Youth assent forms--All youth and their parents or legal guardians are required to complete and sign the youth assent form. Parents or legal guardians must give written permission for youth to participate in filling out the written survey.

Adult consent forms--We also have a consent form for adult participants, also giving their consent to participate in the evaluation by filling out a survey.

Please make sure participants fill out either a youth assent form or an adult consent form along with the survey. Please keep them separate from the completed surveys so we don’t compromise the confidentiality of the surveys.

We want to stress the importance of filling out the unique code and doing so correctly. We ask that you walk the participants through filling out the unique code when they administer each survey.

FIRST letter of your first name / FIRST letter of your middle name / LAST letter of your last name / MONTH of your birth date (Example: Jan=1,
Feb=2, etc.) / DATE of your birthdate
(Example: 1st, 3rd, 27th)
B / J / L / Oct. or 10 / 5

PLEASE COMPLETE YOUR UNIQUE CODE:

It is important to note that the first two boxes ask for the first letter of the first and middle name, and that the middle box asked for the last letter of their last name. The last two boxes ask for the month they were born and the day (not year) they were born.

This code is used to get an accurate comparison between the pre-survey responses and the post-survey responses. This code is completely anonymous. If the participant is uneasy about filling out the code and confidentiality, they may tape the survey shut.

We also want to stress the importance of taking the survey and responses seriously. The results of these surveys reflect your community’s participation and will help determine whether or not this program is extended to other communities in the future. Unfortunately, there were a few Study Circles surveys that had to be voided due to inappropriate answers in a few of our communities.

Important Dates to Remember!

Pre-surveys are due Tuesday, May 8, 2007, (you can send them in any time before that)

Post-surveys are due July 10, 2007.
