Credentialing Exam: / Pharmacy Technician Certification – CPhT
NOTE: NOTE: Candidates must be within no more than 30 days of successful completion of all requirements needed to obtain a high school diploma or the equivalent. Complete details regarding eligibility requirements certification may be found in the candidate handbook on page 9.

The completed application can be faxed to Ms. Austin at 913-661-6280. CPhT testing must be completed at a third party testing site.
Once the application process has been completed, NHA will provide additional instructions regarding student registration and the third party scheduling process.
Proctoring Guidelines: / CPhT testing must be completed at a PSI Testing Center. Please work directly with designated NHA representative, Ms. Tricia Austin. Ms. Austin will handle the scheduling of tests at PSI for your school site location.
