JOB PURPOSE:

To support the work of the Up and Active Operational Team in the planning and delivery of the Up and Active service and other health and physical activity interventions and programmes, with a focus in Rossendale.

MAIN DUTIES AND RESPONSIBILITIES:

1.  To encourage and empower individuals to modify their lifestyle and undertake regular physical activity and dietary changes leading to long-term health improvements.

2.  To deliver a variety of physical activity and Weight Management programmes, e.g. structured Weight Management programmes, Weekly Weigh-Ins, Drop Ins and Cook and Eat sessions for identified individuals across the life course. These include pregnant women, children and young people, families, adults, particularly those socially disadvantaged, and in a variety of community settings, including leisure centres, schools and colleges, community venues and workplaces with a focus in Rossendale.

3.  Targeted interventions: deliver telephone assessments to clients who are inactive and signpost these clients onto appropriate physical activity/weight management interventions. Early intervention: interaction on a face to face basis to identify and support individuals that require the service.

4.  To monitor the progress of clients in relation to activity and weight management; individual goal setting around physical activity and weight loss. To provide ongoing support and follow up to maintain weight loss.

5.  To maintain rigorous standards of record keeping and compliance with Lancashire County Council and local requirements for activity monitoring and evaluation. To contribute to quarterly and annual reports indicating the programme’s effects and efficiency as requested by the Up and Active Development Officer.

6.  Contribute to the marketing, publicity and promotion of the Up and Active service in different settings e.g. attending health/roadshow events and promotions; giving talks and presentations to groups and meetings as required; provide MOT health checks.

7.  Act as an advisor for enquiries and respond to requests for information from clients, partner agencies and other professionals.

8.  To deliver a succession of exit routes to encourage adherence to lifestyle change(s) and provide ongoing support opportunities for social interaction.

9.  To contribute to the recruitment and support of volunteers/health mentors who have a key role to play in supporting the success of the Up and Active service.

10.  To contribute to the delivery of mass participation health and activity promotional events, to engage a wide range of people e.g. Couch 2 5K.

11.  To be responsible for the appropriate mentoring of income and expenditure, including collection of monies, fees and charges, sponsorship and fundraising etc.

12.  To provide support and information, when required, to the Up and Active Operational Team as appropriate to ensure the continued success of the service.

13.  To attend such meetings, training or carry out other appropriate duties as may be required from time to time by the Operational Team in line with the grading of the post.

OTHER INFORMATION:-

Due to the nature of the work, flexible working, regular evenings and weekend work is a feature of this job. You may be required to deliver services throughout East Lancashire and not at your main place of work. This is reflected in the salary for the post.
