This registration form enters you as an applicant for The Fashion Model Search at SANTANA ROW on Saturday, September 13th, 2008. Model Search will be held at Santana Row is the via of Santana Heights, located between SINO and Left Bank Brassiere. Males and females ages 18 to 30 years old are invited to walk the runway. No modeling experience required. Please bring THIS FORM and a recent or professional photo (non-returnable) with you to the model search on Saturday, SEPT. 13th.

You must attend the model search on SEPTEMBER 13th to win! Only ONE contestant will be awarded the grand prize and notified on Sept 15th. Please complete the form in its entirety and your signature is required. Winner will receive: A 1-year modeling contract w/Halvorson Model Mgmt, a fashion photo-shoot with Frank Anzalone- top Bay Area photographer, a runway coaching session and featured in an upcoming Santana Row Fashion show.
