Dear Parent,

The band will be taking educational and performance field trips during the school year. Permission must be granted by parents for the student to be able to accompany the group. Please completely fill out and sign this MANDATORY form.

Field Trip Permission

My son/daughter, ____________________________________, has my permission to go on all field trips scheduled for the LHS Performing Arts Department during the 2014/2015 school year. Participants agree to abide by applicable rules and standards of conduct as set forth by FCPS and Linganore Band Boosters, Inc., and release and hold harmless these organizations, their agents, volunteers, and/or employees from any and all claims of liability arising out of this participation.
