I, _________________________________, an employee of the Sac and Fox Nation, authorize the amount of $______________ to be deducted from my payroll check each pay period for the purpose of participation in the Employee Fund. I understand the Employee Fund is governed by the Employee Committee and monies paid into the Employee Fund are used to purchase coffee, cups, crème, sugar, hot chocolate, apple cider, flowers, greeting cards, birthday cakes, and for activities on special occasions. I also understand it is not mandatory to authorize a payroll deduction for the Employee Fund.

Authorization for this payroll deduction can be withdrawn at any time by the Employee submitting a written notification to the Human Resources Department with a copy provided for the Accounting Department.
