· The Fashion Show on Thursday, February 25th (5:30-9:00)

I will be on time and I will stay for the entire duration of the show. I also agree to sell a minimum of 5 tickets to friends, faculty, coworkers and family.

To the Parent/Guardian:

If your daughter is selected as a model for the Junior/Senior Prom Fashion Show, your daughter will be responsible for the care of the merchandise while it is in her possession the night of the Prom Fashion Show. Windsor Store located in Woodbridge Center will require a credit card from each model, so that a mock receipt will be produced the day the model goes to be fitted for a dress. If any dry-cleaning resulting from perspiration, makeup, etc. will be billed to the credit card. Any damage to the merchandise will be paid for in full by the individual model.

A signature by you indicates you have read and agreed with the conditions set forth by Windsor Store and allow your daughter to be a participant in the Junior/Senior Prom Fashion Show.
