Payment Options: check, cash, credit card, or Bank Transfer; due before course. There will be no one permitted in the course unless and until fees are fully paid.
Course fee includes a copy of both Practical Design of Optical Thin Films, and Practical Production of Optical Thin Films which cover essentially all of the material presented (see web site), daily morning and afternoon coffee breaks, and the Course Banquet on 7 December 2017.
You are asked to bring your own laptop with Windows and Excel installed for use in the course in order to gain the full benefit of the in-class use of the free FTG Software and the software provided by Willey.
Please return a copy of this form and answer the questions below as soon as practical, even though the fees can be sent at a later time.
Refund of Fees: If a paid registrant gives notice of withdrawal before 29 September 2017, full costs will be refunded less a $100 handling fee. If a paid registrant gives notice of withdrawal after 29 September 2017 but before 31 October 2017, 50% of costs will be refunded. There will be no refund for non-participation and no notice by 31 October 2017.
TRAVEL: Charlevoix is about 50 miles North of TVC Airport (Traverse City, MI) and the same distance South of PLN (Pellston, MI). Both are about a $100 taxi ride each way, so a rental car is advised. TVC has many more connection options from DTW (Detroit, MI) and ORD (Chicago, IL). Delta, American, and United Airlines serve TVC. Only Delta serves PLN.
LODGING: The Edgewater Inn, where the course sessions are held, is the best value in town.
