201 5 Employee Benefits Summary

Vacation: Exempt employees earn three weeks of vacation time per year. Beginning in year two an additional day is accrued so at the beginning with the fifth year of service the employee is accruing four weeks per year.

Holidays: Employees are paid for 10 holidays, one of which is a floating holiday.

Sick/Personal Leave: Regular full-time employees are eligible to accrue up to a total of 80 hours of paid sick/personal time each benefit year.

Sick/personal time may be used for an illnesses and doctor or dental appointments for the employee and immediate family members. Two personal days are included in the 80 hour accrual and may be used to attend to personal matters outside the office with advanced approval. Unused personal time is converted into sick time, and carried over from one benefit year to another with a maximum carryover from year to year of 13 weeks, or 520 hours, for regular full-time employees.

Exempt employees are required to take time off in full or half day increments.

Funeral Leave: Employees may be eligible to take up to three (3) working days off without the loss of salary to attend a funeral.

Jury Duty: Employees may be paid if he/she is required to report to jury duty.

Transportation: Every month employees receive a $50 stipend to assist in commuting costs to and from the office to be used in conjuction with the following; PACE suburban buses, METRA commuter trains, Ventra passes and rapid transit. This is a tax free benefit.

Medical: Benefits are offered through Blue Cross/Blue Shield.

Annual deductible per individual for the PPO is $500. Lifetime Benefit Maximum is Unlimited. Most services are covered at 80% if provided by a PPO network physician or at a PPO network hospital. Otherwise, services outside the network are generally covered at 60%.

The HMO plan includes a $100 in-patient hospital co-payment with an unlimited major medical maximum. Most general services are available at no cost/$20 co-pay to the individual.

Dental: Dental insurance is a traditional plan offered through MetLife. The annual deductible per individual is $50.00 with a calendar maximum of $1,800 in benefits. The employee contribution per pay period is:

Vision: Vision insurance is available through VSP member doctor network. The employee contribution per pay period is:

401(K) Plan: The AHIMA 401(k) Plan is for the exclusive benefit of all eligible employees and their beneficiaries. The plan allows eligible employees to defer a portion of their income on a tax- favored basis and/or to receive an employer contribution. New hires will be eligible the first of the month following their date of hire. AHIMA contributes up to a 5% match on employee contributions. The match is dollar for dollar and employees are vested 100% on their first day of eligibility. Our plan includes an automatic enrollment feature at 3% with automatic annual increases in your contributions (payroll deductions). Employees have the ability to opt out of the automatic enrollment and automatic increase. Any contributions for which you do not provide investment direction will be invested in the Plan’s designated default funds based on the assumption that you will retire at 65 yrs.

Eligibility: Eligible employees can participate in the 401k program on the first day of the month following hire date.

Fitness Center: Chicago employees have access to the workout center located on the 23rd floor. Equipment includes premium cardio equipment, weight training machines, exercise mats and other ancillary equipment. Men’s/Women’s shower and locker room facilities and TV monitors on most equipment. Hours of operation are 6am-8pm; Mon-Fri.

Tuition Reimbursement : Employees are offered a strong career development program in order to improve job satisfaction, provide opportunities for skill building and career development.

Employee professional development banks: Planned training dollars are also made available to employees for professional development opportunities in order to gain the knowledge and skills that add value to AHIMA's mission and are personally beneficial.

University of AHIMA: The mission at The University of AHIMA is to prepare staff for challenging and rewarding job assignments. Courses provide training in knowledge areas identified as valuable across a wide spectrum of disciplines. The University’s objective is to ensure employees posses the necessary skills and knowledge needed to exceed member and customer expectations.

Flexible Spending Accounts: Employees are eligible to participate in TASC flexible spending account plans to set aside pre-tax dollars for medical , dependent care, travel, and parking expenses.

Employee Assistance Program (EAP) for employees and family members

Life/AD&D: Benefits are offered through Cigna. The plan provides life insurance benefits of one (1) times employee earnings. This plan also provides for benefits in the event of accidental death or dismemberment. The premiums are paid by AHIMA. Eligibility: First day of the month following the employee date of hire.

Voluntary Life: Employees are eligible to elect voluntary life insurance for themselves and dependents.

Short Term Disability: Short term disability benefits are available after a seven (7) day waiting period. This coverage pays the insured 60% of his/her weekly salary for a thirteen (12) week period.

Long Term Disability: This coverage pays the insured 60% of his or her monthly salary up to a maximum of $15,000 per month in the event of a total disability. The waiting period before benefits begin is 90 days.

Travel Accident: AHIMA provides travel accident coverage to AHIMA employees traveling on AHIMA business.

Short Term Incentive P lan : Eligible employees participate in the AHIMA incentive plan based on date of hire.

Rewards and Recognition Program: Employee centered reward and recognition program designed to individual and team growth, collaboration and organizational success.

Voluntary benefits – Annual CPR/first Aid certification, Influenza immunization, Global Fitness membership.

Social Security (FICA). This fund is intended to provide the employee with retirement, long-term disability payments or survivors’ benefits in the event of an employee’s death. This program is based on a percentage of earnings each year, and AHIMA matches the employee's contributions. The cost to AHIMA for this benefit is approximately 46% of payroll costs.

Workers' Compensation Insurance: This benefit provides for medical expense and work time lost because of on-the-job accidents or injuries. AHIMA pays for the entire cost of this insurance. The cost to AH IMA for this benefit is approximately 2% of payroll costs.

Unemployment Compensation: AHIMA pays the entire cost established by federal and state governments to provide unemployment compensation for its employees. The cost to AHIMA for this benefit is approximately 3% of payroll costs.

Employees receive paychecks every other Friday for a total of 26 paychecks per year.

AHIMA's office dress code is business casual. Mondays and Fridays are casual. The normal work week is 40 hours/8 hour work day, this excludes a lunch break (30min-60min). Flextime is also offered.
